Welcome to PhysioFit PT & Wellness (“PhysioFit,” “we,” “our,” or “us”). By using our website, booking an appointment, or receiving services, you agree to the terms outlined in this Service Agreement (“Agreement”). Please read this carefully, as it governs your relationship with us.

1. Scope of Services

PhysioFit PT & Wellness provides health and wellness services, which may include:

• Physical Therapy & Occupational Therapy (injury recovery, post-surgical rehab, chronic pain care)

• Pilates & Movement Rehabilitation (core strengthening, posture correction, mobility training)

• Myofascial Release (therapeutic massage, craniosacral therapy, recovery support)

• Lifestyle Medicine Programs (exercise, nutrition, toxin reduction, pain management)

• Hypnotherapy & Stress Management

• Educational Workshops & Online Courses

Services may be delivered in-person at our clinic, virtually, or through online platforms.

2. Client Responsibilities

By receiving services, you agree to:

• Provide complete and accurate health and personal information during your intake.

• Inform your provider about any changes in your medical condition, medications, or restrictions.

• Follow your care plan, recommendations, and instructions to the best of your ability.

• Use our website and booking systems in a lawful and respectful manner.

3. Payment & Billing

• Payment is due at the time of service, unless otherwise arranged.

• We accept cash, debit/credit cards, and other approved payment methods.

• Packages, memberships, or prepaid sessions must be used within the stated time period.

• Missed or canceled appointments without at least 24 hours’ notice may be charged the full session fee.

4. Insurance

7. Risks & Acknowledgement

You understand that:

• Participation in physical therapy, exercise, massage, or wellness services carries some inherent risks.

• Results vary and are not guaranteed.

• You should consult your physician before beginning any new treatment, exercise, or wellness program.

8. Privacy & Confidentiality

9. Limitation of Liability

• Payment is not made when due.
